Conditions of Employment

Following an offer of employment by the Director of Human Resources, all new employees will be asked to complete the following forms:

• Employment Eligibility Form (I-9) -- This form must be presented to Human Resources with evidence of identity (for example, a current driver's license or Division of Motor Vehicles (DMV) photo identification card and Social Security card or passport) within three (3) days after employment.
• Criminal Conviction Investigation Form -- This form authorizes a criminal history search of the Central Criminal Records Exchange.
